A national clinical study at 11 centers including Primary Children's Medical Center will try to determine which of the two is the safer, more effective treatment for those children with status epilepticus, an unrelenting spate of seizures that can cause brain damage and even death. In a Lorazepam and diazepam study of 38 pregnant women with histories of panic disorder who were treated with 0. 5 mg of clonazepam during pregnancy we found no evidence of neonatal toxicity or withdrawal syndromes in their babies (Psychother. Recent animal work in both guinea pigs and rhesus monkeys has shown that midazolam is significantly both more potent and more rapidly acting in controlling soman-induced seizures than is diazepam or lorazepam and diazepam lorazepam when given by this route. Outcomes measured The lorazepam and diazepam primary outcome was successful abolishment of seizure activity within 20 minutes of the initiation of drug infusion without recurrence in the next 40 minutes (1 hour total from beginning of infusion). The researchers are hoping to study 240 children, divided about equally between the two medications.

Because no one can predict when a child will have a seizure and because the time to treat it in the emergency room is very short -- about five minutes to get one or the other medication on board -- the study has an unusual enrollment/opt-out. Benzodiazepines have many different names and there are different dosages, but chemically they are all in the same family and come with similar risks: Ativan (lorazepam), Serax (oxazepam), Rivotril and Klonopin (clonazepam), Xanax (alprazolam), Valium (diazepam), and many more, are all highly addictive substances that have serious health consequences over long-term usage. But the incidence of associated neonatal complications is probably extremely small. All 4 treatment groups lorazepam and diazepam were similar in baseline characteristics. Of these three drugs, phenytoin is ineffective against nerve agent-induced seizures, it could not be given by the intramuscular (IM) route (a necessity for self/buddy aid of a nerve agent casualty), and pentobarbital produces anticonvulsant effects against nerve agent seizures at only near-anesthetic doses. It can be triggered by epilepsy or by the type of seizure that results from high fever, low blood sugar, a central nervous lorazepam and diazepam system infection or head injury. All drugs used to treat nerve agent casualties must be Food and Drug Administration (FDA) approved, and under terms of the Department of Defense budget signed September 1999.
The authors are correct to point out that soldiers pretreated with pyridostigmine are much more likely to go lorazepam and diazepam into prolonged status epilepticus when exposed subsequently to nerve agents and promptly treated with atropine and 2-PAM CI (MARK 1), rather than die, than are soldiers not so pretreated. Discontinuation of antipanic drugs during pregnancy appears to be associated with a high risk of relapse, which lorazepam and diazepam may lead to compromised perinatal outcome. Exclusion criteria included pregnancy, treatment prior to enrollment with an anticonvulsant and no further seizure activity, a neurologic event necessitating surgery, contraindication to treatment with lorazepam and diazepam study drugs, or status epilepticus of a type different from generalized convulsive status epilepticus.

One is approved for children, the other not, but both have been used for years.

The National Institutes of Health on Wednesday hosted a teleconference to explain what will be a large-scale national study to determine which medication works better for lorazepam and diazepam children with the condition, which even with treatment is fatal in as many as 4 percent of cases.
